Our providers and staff at STEM hold the utmost respect for the Geriatric patient. We are here to guide them and their families through the process of planning and coordinating care for the elderly with physical and/or mental impairments to meet their long term care needs. We work closely with families to improve their quality of life and maintain their independence for as long as possible.
Functional Decline:
We provide a team approach as we plan for the functional decline in older patients. This includes fall risk screenings, fracture prevention, orders for assistance devices, and referrals to Physical Therapy when needed to help with balance and coordination. We have staff available by phone 24/7 for those falls or mishaps when they occur. Our facility provides x-rays on site and Dr. Jondy is able to provide the care for fractures if needed.
Social Needs:
Social needs of the elderly can include help with meals, transportation, social events, or finances. We provide the resources needed to ensure these needs are met for your loved ones.
Emotional Needs:
Emotional support of the elderly is another concern but not a lesser concern. We provide regular depression screenings and baseline memory testing. We communicate closely with the family to improve their day to day life and will provide outside resources when needed. We are experienced in transitioning the patient from their home setting to a skilled nursing home when the need should arise.
